• Transparency in Health Coverage

When a district employee is injured at work, it is important to report the incident right away by calling the RAS Work Injury Hotline.  Below are documents outlining the reporting procedures for Employees and Supervisors/Health Office staff.  

Report the incident immediately to your supervisor/health office and follow these steps :

1. For emergencies, supervisors/health office staff will call 911
2. For non-emergency situations, call the RAS Work Injury Hotline at 1-855-736-9482 (available 24 hours/day, 7 days/week)
   • During business hours (M-F, 7:00-4:00), you will be prompted to choose:
     • Option 1 – use this option to report incident only (no medical treatment needed)
     • Option 2 – use this option if unsure if medical treatment is necessary After business hours, a registered nurse will answer all calls
   • You will be asked to answer the following questions:
     • Your company name: South St. Paul Public Schools o Building location:
       • High School or Middle School, 700 North Second Street
       • Lincoln Center, 357 9 th Avenue North
       • Kaposia, 1225 First Avenue South
       • Community Learning Center, 710 19 th Avenue North
       • Family Ed Center, 104 5 th Avenue South
       • District Office, 104 5 th Avenue South
     • Describe the incident to complete a First Report of Injury Form
   • For option 2 calls, a registered nurse will ask questions to determine if medical treatment is recommended and direct you to visit a designated clinic, if necessary (listed below)
     • Allina Health Clinic, 5565 Blaine Avenue, Inver Grove Heights
     • HealthPartners, 205 Wabasha Street South, St. Paul
     • MN Occupational Health, 1400 Corporate Center Curve #200, Eagan

If Medical attention is necessary:

• Obtain a Workability Report (doctor’s note) at each clinic visit. You must submit a Workability Report to your supervisor within 24 hours of each visit.
• If the health care provider indicates you have work restrictions, your supervisor will review your Workability Report with Human Resources to determine if your restrictions can be accommodated.
• If the health care provider indicates you are unable to work, you must communicate with your supervisor.

If an injury to an employee cannot be reported by calling the Hotline, the following First Report of Injury Form must be completed and submitted to Human Resources within 24 hours of the incident.

Changes to health and/or dental insurance outside of the open enrollment period may be possible if a qualifying life event occurs.  Please contact Barb Lamfers in Human Resources for more information.   Required paperwork for coverage changes, as a result of a qualifying life event, must be submitted to Human Resources within 30 days of the life event.

Below is additional information pertaining to specific qualifying life events:

Obtain New Coverage

If you, or your dependents, have obtained new insurance coverage within the last 30 days, you can drop coverage. You must provide evidence of new insurance coverage, which must show the name(s) of the individuals who have new coverage AND the date the coverage began. 

Marriage

You may change your insurance coverage from single to single+1 or family coverage by adding your new spouse (and his/her eligible dependents) to your plan. Coverage for your new dependent(s) becomes effective on the date of marriage. 

Divorce

In the event of a divorce, the ex-spouse is no longer a legally married spouse and coverage must be dropped within 60 days of the divorce date. The ex-spouse will be offered MN Continuation of Coverage through COBRA.

New Child

Coverage becomes effective on the date of birth or adoption. 

Involuntary Loss of Coverage

If you, or your dependents, have involuntarily lost coverage within the last 30 days, you can add eligible dependents to your plan (if you initially waived coverage, you can enroll in coverage). You must provide evidence of loss of coverage. Evidence may be a copy of the COBRA notice, HIPAA notice, or some other employer or insurer document that clearly shows the name(s) of the individuals who lost coverage AND the date the coverage ended. Documentation must be forwarded to Human Resources within 30 days of the loss of coverage. 

Child Graduates from High School or College

This is not a life event. You can remove a child from your coverage if he/she has obtained their own coverage within the last 30 days (see Obtain New Coverage above), if the child turns 26, or during an open enrollment period.
