Application Process

Complete the Statewide Enrollment Options Form and return to Student Information Services via email to studentinfo@sspps.org or mail at:

An application must be completed for each student. The parent/guardian address section must contain an address in the state of Minnesota.

Please note the information contained within Policy 509 concerning admission. Open Enrollment legislation requires that you discuss your reason(s) for leaving with an assigned staff person in the school your student is currently attending. Usually this is with a counselor or principal.

Resident District Notification

Once your application has been reviewed, you will be notified on your open enrolled status by building staff.
